Service Development (Projects/Strategy)

Counties Manukau District Health Board (CMDHB) has outlined the actions it plans to take from 2006 through to 2010 in order to meet the health and disability needs of older people in Counties Manukau.  The founding document underpinning this Action Plan is the national Health of Older People Strategy (2002).  The vision of the Strategy is that:

“Older people participate to their fullest ability in decisions about their health and well being and in family, whanau and community life.  They are supported in this by coordinated and responsive health and disability programmes.”

The NZ Health of Older People Strategy sets out the government's policy for the future direction of health and disability support services for older people. It provides a vision of older people participating fully in decisions about their health and wellbeing and in social and community life. If health and disability support services are to play their part in enabling older people to live this vision, they need to focus much more on:

Older people participate to their fullest ability in decisions about their health and wellbeing and in family, whaanau and community life. They are supported in this by co-ordinated and responsive health and disability support programmes.

CMDHB Achievements to date

  • Franklin Integration Project
    Committee established.
    Transport & Pamphlet surveys completed.
    Older persons expo held 29 September 2006.
    Older persons expo held 5 October 2007.
     

  • Stroke
    Stroke service and way forward document completed.
    Stroke Guidelines project completed and the document presented to DiSAC and then circulated for comment. It is intended that the recommendations will be incorporated into service planning. The four recommendations are: Designated stroke leadership, Co-ordinated outpatient follow up, Consistent protocols across care settings, and Culturally appropriate support.
     

  • Disability services
    CMDHB Disability Action Plan completed.
     

  • Health Of Older People
    Health Needs Analysis completed.
     

  • Disability Strategy Coordinator
    Matthew Slade was appointed to this position in January 2007.
     

  • CBRT (Community Based Rehabilitation Team)
    Team established.
     

  • Ortho-geriatric service
    Evaluation and recommendation document completed.
     

  • Accreditation of Rehabilitation Service
    First Australasian rehab service to undertake tele-accreditation as a training institution of the AFRM (Australasian Faculty of Rehab Medicine).
     

  • Multidisciplinary EDS in AT&R
    Medicine reconciliation audit AT&R.
    AT&R pilot Te Kanga best practice.
    Established succession planning for rehab consultants.
     

  • Rehabilitation Scoping study
    Study completed and recommendations for the clinical services model and service delivery model.
    Joint document with ACC.
     

  • Needs Assessment & Service Coordination (NASC)
    Evaluation and way forward document completed.
    One stop shop NASC concept implemented.
    Integration of short term home help, meals on wheels, mental health into NASC.
